Practice
Please take note of the following practice guidelines and fees:
Appointments:
- Please call the office manager at 504-891-8808 to schedule, change or cancel your appointment.
- The office will most often call you to remind you of your appointment, yet, please be mindful of keeping your appointments.
- Cancellations: Please make cancellations one or more working days in advance when possible, by calling 504-891-8808 or e-mailing Lisa Gulldge at lgulle@att.net. Cancellations made less than a working day in advance will be billed in full.
- No-shows: No show appointments will be billed in full.
Payments and Procedures
- Payments should be covered in full at the time of your visit. Mindful Living Program© fees are paid in full at the beginning of the course although other arrangements can be made on a case-by –case basis. Mindful Psychiatry accepts credit cards, personal checks and money orders.
- Mindful Psychiatry will keep a copy of your credit/debit card and for billing purposes only. Such copy will be safe-kept confidentially.
- At present, Mindful Psychiatry does not participate with any private insurance, Medicare or Medicaid. Please contact your insurance carrier about out-of-network benefits and co-payments.
- Mindful Psychiatry will give you a receipt at the end of your visit with all the information necessary should you wish to submit it to your insurance for reimbursement. The information on your receipt has personal and confidential data necessary for billing insurance companies including: Date of service, your full name, date of birth, name of diagnosis and diagnostic ICD code and, type and length of service provided using CPT codes.
- Past-due accounts for more than 30 days will incur a late fee of 1.5% per month. Should collection/legal proceedings become necessary to recover a past due account, the charge associated with such procedures –i.e. collection fee, court cost, attorney’s fees, etc. will be the sole responsibility of the patient who incurred the debt.
- Prescription Call-ins: Please make sure you communicate your need for medication refills at the time of your visit. Excessive call ins’ for medications will be charged a fee of $10.
- Controlled substances early refills: In general, Dr.Calderón does not provide early refills for controlled substances that will run out before time. Emphatically we advise to refrain from making adjustments to your dose on your own without discussing this with Dr.Calderón during your visit.
- Urine drug screens: Urine drug screens are helpful monitoring tools and standard of care in addiction treatment. Some patients are required to comply with random drug screens as part of their treatment. Failure to submit a urine drug screen within 48 hours of request will be taken as a breach in your treatment contract and addressed on a case by case basis.
Fee Schedule
Starting February 1st, 2011 there will be change in my practice fees. Initially, the new fee will only apply to all new patients. Fees for existing patients will continue at current rates until April 1, 2011 when I will use the same fee rate for everyone.
The increases in fees reflect a higher than expected operating cost for the office and higher malpractice insurance for the year.

- Psychotherapy w/medication management 25 min. $125
- Psychotherapy w/medication management 50 min. $200
- Medication management and therapy 30 min. $148
- Family Therapy 30 min. with patient $150
- Family Therapy 30 min. without patient $125
- Mindful Living Program© Stress Reduction Groups 90 min. $40-60/session sliding scale
- Suboxone Induction*
- Medication management 20 min. $108
* Suboxone induction and related visits are charged as regular practice visits as above. Some additional charges for extensive telephone contacts during induction may incur.
Communications
- Electronic and wireless communications via E-mail, or cell phone are not entirely secure and can potentially be seen by unauthorized people. Please refrain when possible from disclosing personal, private or sensitive issues using E-mail, text messaging or cell phone.
- If you need to leave a non-emergency communication, please call the office and leave a message use or send an e-mail. These will be returned within one-two business days.
- For personal emergency communications you may call the office at 504-891-8808, Dr.Calderon’s cell phone 504-220-8336; you may call 911 or go to your nearest Emergency Room. Do not use e-mail, page the cell phone, or text message for personal emergencies.
